Many insurance plans cover testosterone replacement therapy (TRT) once eligibility is confirmed through medical evaluation. Typically, insurance providers require two separate blood tests showing that testosterone levels are below 300 nanograms per deciliter (ng/dL) to qualify for coverage. These tests ensure that the patient genuinely needs the therapy and helps streamline the approval process.
